Request a Meal

A meal request will be sent to the Meals Team who will cook and deliver meals to those within the SLC family due to various personal seasons of need. By submitting this form, the one requesting meals and the one receiving meals understands that this is for personal care from laypersons of SLC and will not hold SLC or those providing care liable to any degree. Upon submission of this form, the information will be evaluated and a SLC representative will be in contact. Thank you for the opportunity for SLC to serve you and your family in this manner.
