Request a Visitation

A visitation request will be sent to the Visitation Team consisting of trained volunteers desiring to visit with those who are homebound or hospitalized locally to pray together, read Scripture, and bring encouragement to individuals and families. By submitting this form, the one requesting visits and the one receiving visits understands that this is for personal care from laypersons of SLC and will not hold SLC or those providing care liable to any degree. Upon receiving this form, a SLC representative will be in contact. Thank you for the opportunity for SLC to serve you and your family in this manner.
